Headstone Transcript Form
We purchased a cemetery packet at a genealogy seminar. In it was a form to use to jot down (transcribe) the information available at the cemetery and on the headstone. Much of the information is not needed for every cemetery, or you might just not be interested in that item (such as what the headstone is made of).
I created a blank form--a simple Word table--and duplicate it for each person we hope to find on that trip. I type in the name and the FindAGrave number (in case I want to easily look them up on the road). I also have pages of blank forms (no names added) in case we find someone that we did not know was in that cemetery. Below, I have also included a Image of the form, and a downloadable PDF.

- LAST, First Middle (maiden, first husbands surname, other surnames used)
- FindAGrave # (if already in that database)
- I type this information in for each person I know is at the cemetery
- Cemetery Name
- I type this in for each person's form we'll be looking for
- Cemetery Address
- I type this in for each person's form we'll be looking for
- Birth Date
- Death Date
- Records Location
- Grave Location
- Section, Plot, Site, Range (any way of finding it again)
- Date(s) Interred
- Sexton
- Marker Text
- Headstone Text (front):

- Also include if there are name/initial corner stones
